Clinic Policies

  • Albertans can register for Alberta Health Care & update contact details here: Alberta Health Care Insurance Plan
  • New patients should make an initial meet and greet appointment to set up a new doctor patient relationship (bring health care card & photo ID).
  • We follow the usual medical model to make a diagnosis i.e. obtain a history of the problem from the patient, then do an examination and then come up with a working differential diagnosis and then formulate a plan of management which may include further investigations, treatment and possibly referral for specialist opinion and management.
  • We will deal with the most pressing complaints at each visit as time allows and you can book follow up appointments as necessary.
  • Prescriptions:  your doctor needs to see you to assess your medications at regular intervals. We will occasionally fill requests for faxed prescriptions, however there will be a $30 charge for this, as this service is not covered by AHS.
  • Payment for uninsured services or missed appointments is due in 30 days. Refer to the uninsured services notice on the FAQ page.
  • Please make a follow-up appointment for the results of all investigations.
  • The clinic will notify you as soon as we receive specialist appointment bookings.
  • Your doctor will provide thorough prenatal care up to about 24 weeks gestation, and will arrange follow up at a prenatal clinic.
  • Newborn babies should be seen at 1 week, 2 weeks, 1 month, 2 months, then 4, 6, 9, 12, 15, 18 months (9 and 15 months can be skipped for well babies).
  •  If your call is not answered, then please try to call later or book an appointment online. Our telephone lines are answered from 8:00 a.m. to 5:00 p.m. Our telephones do not take messages.

Beltline Medical Clinic's physicians are members of the Calgary West Central Primary Care Network (CWC PCN). The PCN offers the following programs in partnership with your family doctor and at no cost to you:

  • Access to a chronic disease management (CDM) nurse. Unfortunately, at present, the Behavioural Health Therapist, Physiotherapist and Pharmacist service programs have all been terminated by the West Central PCN.
  • Access to the Calgary West Central Primary Care Centre at 5960 Centre Street SE  with a same day appointment service (Tel 403 249-9907 or call Alberta Healthlink 811). Available from 9 a.m. to 9 p.m. weekdays and from 1 to 4 p.m. on weekends, This service is by appointment only, is available 365 days a year and provides an alternative to the walk-in system or a visit to the ER.
